It is our wish that readers discover the importance of polymyxin structure in relation to the mechanisms of activity, resistance and toxicity. We emphasized that reliable analytic methods for polymyxins are critical when investigating their pharmacokinetics (PK) and pharmacodynamics (PD). The complicated dose definitions and different pharmacopoeial standards have already compromised the safe use of polymyxins in patients. Therefore, informed by the latest pharmacological information, scientifically-based dosing recommendations have been proposed for intravenous polymyxins. Considering the PK/PD limitations and potential development of resistance, polymyxin combinations are encouraged; however, the current literature has not shown definite microbiological benefits, possibly because most clinical studies to date overlooked key PK/PD principles. Nephrotoxicity is the major dose-limiting factor and it is imperative to elucidate the mechanisms and develop novel approaches to minimize polymyxin-associated toxicities. In addition, the anti-endotoxin effect of polymyxins supports their clinical use to treat Gram-negative sepsis. Fortunately, the discovery of new-generation polymyxins with wider therapeutic windows has benefited from the latest achievements in polymyxin research.

The emergence of multidrug-resistant (MDR) Gram-negative 'superbugs' coupled with a rapid decline in the development of new antibiotics has led to a serious, global health crisis. Polymyxin antibiotics, which include polymyxin E (colistin) and polymyxin B, were introduced over 50 years ago and were seldom used due to nephrotoxicity. However, recently, the polymyxins have re-emerged in the clinic as an antibiotic of last-resort. There are increasing reports of polymyxin resistance and failure during polymyxin monotherapy. While polymyxin based combination therapies are a promising alternative, there are significant gaps regarding the optimal use of such combinations. The primary goal of this dissertation is to rationally optimize polymyxin combination therapy based on pharmacokinetic/pharmacodynamic (PK/PD) principles. Menchanism-based (PK/PD) models (MBMs) were employed to evaluate mechanisms of antibiotic synergy. A secondary goal is to define the mechanisms of polymyxin tolerance and resistance utilizing genetically modified knockout strains and to develop MBMs that quantify the impact of bacterial regulatory systems including quorum sensing mechanisms on antibiotic PD.

The first book was on "Theory and Practice" of antibiotic stewardship in its broadest sense -the how to do it and the do's and don’ts. The second, on "Controlling resistance" was very much on the relationships between use and resistance and beginning to home in on the hospital as the main generator of resistance, but mainly looking at it from a disease/clinical perspective. The last 3 chapters on MRSA, ended where the 3rd book will take off. "Controlling HAI " will concentrate on specific MDR organisms highlighting their roles in the current pandemic of HAI and emphasizing that the big issue is not so much infection control but antibiotic control, in the same way that antibiotic over-reliance/ over-use has caused the problem in the first place. Up 'till now the emphasis for controlling MRSA, C diff and all the other MDROs has very much been on IC, which clearly isn't working. This book will gather all the evidence for the increasingly popular view that much more must be done in the area of antibiotic policies/ stewardship, especially when we are in danger of a "post antibiotic" era, due to a real shortage of new agents in the pipeline.
