Many Americans believe that people who lack health insurance somehow get the care they really need. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Statistics published by the U. S. Department of Commerce (1980) indicate that in 1977 we spent 8. 1% of our gross national product (GNP) on life, health, property-casualty, and other forms of insurance. An additional 5. 7% was used to pay the Social Security tax, which is another form of insurance premium, for a total of 14. 8% of the GNP. \ Although insurance had its historical origin in marine insurance, it has now developed into one of the major industries of the American economy and extends into many areas of economic activity. One area where growth has been particularly strong is the medical sector. Health insurance is a major institution in all industrialized countries. It became a government responsibility in 1883 when Bismarck intro duced a compulsory program of health insurance for industrial workers in Germany. Programs for workers in various industrial and income categories soon followed in other European countries-Austria (1888), Hungary (1891), Norway (1909), Servia (1910), Great Britain (1911), and Russia and Romania (1912) (Rubinow, 1913:250). Programs in these countries were extended in subsequent years, and other countries in Europe followed with their own programs. Consequently, today most industrial countries have universal or near-universal health insurance coverage. In the United States the issue of national health insurance has been seriously debated since just prior to World War I, and polling data since the 1930s show that a substantial majority of the public has been supportive of such a program (Erskine, 1975).

The burial societies of the Romans were, essentially, private group insurance programs. So were the protection funds of medieval guilds. Largely through the efforts of labor unions, by 1968 more than two-thirds of the labor force in U.S. industry was covered by group life and health insurance plans mostly provided (as fringe benefits) by employers. Today the proportion is even higher, and the establishment of national health insurance, to be sponsored by government, is being debated in the halls of Congress. Complete medical care for the citizenry, with health professionals partly or wholly salaried by a government agency, is now standard in many coun tries, including those of eastern Europe, most of the British commonwealth (including Australia, Canada, and New Zealand), several Latin American countries, Greece, Turkey, Sweden, and of course China, the USSR, and eastern Europe. The major alternative scheme, in which the government provides reimbursement for private care, is employed by several other West ern nations, including Norway, Denmark, Austria, West Germany, and Spain. Both of these methods of government coverage exist for certain groups in the United States: the former for military personnel, service-connected or impecunious veterans, and the indigent mentally ill; the latter for those cov ered under the 1965 amendment to the Social Security Act. However, most health insurance in the United States is private, much of it operating on a group basis.

In Steps on the Road to Medicare Stuart Houston shows that Saskatchewan has led in the development of publicly funded health care since 1915. Among Saskatchewan's many firsts were the payment of municipal doctors, the development of municipal hospitals, and advances in the treatment and prevention of tuberculosis - then the leading cause of death - that culminated in January 1929 with universal free diagnosis and treatment of TB. Given this background of leadership, it was logical for North America's first social democratic government, the CCF, led by Tommy Douglas, to go further, beginning with medical care for pensioners and widows. This was quickly followed by a universal, comprehensive health care plan, instituted in the Swift Current region in July 1945, two years before Britain began such a program. Universal, province-wide hospitalization insurance was put in place in January 1946. Advances in psychiatry consisted of the first inclusion of psychotic patients in an open psychiatric ward in a general teaching hospital in 1955, while cancer firsts included the first government-sponsored cancer clinics, the first full-time cancer physicist, and the world's first use of calibrated betatron and cobalt-60 machines for treating cancer. Why was Saskatchewan so consistently first in health care? Houston argues that not only was the population both altruistic and ingenious with a well-developed spirit of co-operation but that its leaders, including Maurice Seymour, R.G. Ferguson, Harold Johns, and Tommy Douglas, showed unusual foresight. He details how from 1915 through 1962 government responded quickly to public need and suggests that it should be equally responsive today.
